Death Certificates

Each request for a certified or uncertified copy of a death record must be in writing and contain the following information:

• The registrant's full name, signature, & address
• Copy of a Valid Identification
• Date of death
• The county of death
• Parents' names
• Occupation
• Spouse's name (if applicable)
• Reason for the request
• Number of copies needed

CERTIFIED copy: $3.00
UNCERTIFIED copy: 50¢ + $2.00 handling charge
